Title PATIENT-CENTRIC APPROACH IN THE MEDICAL SERVICE SYSTEM OF RURAL AREAS

Abstract The article reveals the patient-centric approach in the medical service system of rural areas, the essence of which is that the target setting of a doctor becomes an orientation not only to rid the patient of a particular disease or to reduce its symptoms, but also to include the patient into developing active practices of maintaining a healthy way of life, independent preservation and restoration of vital forces of his organism. The patient-centric approach in the medical service system leads to a new level of trust. The patient views the doctor as a specialist who is able to understand the combination of factors that affect both the occurrence of the disease itself and the means of overcoming it, as well as ensuring a long healthy state of the organism. Based on a survey of patients in Kurgan (n = 300) and Sverdlovsk regions (n = 300), as well as interviews with medical personnel working in rural areas (n = 16), the article states that the medical service system in rural areas reproduces ideology of paternalism that hinders implementation of the patient-centric approach. This is due to the reproduction and deepening of social inequality, the decrease in the availability of medical service to rural inhabitant, the lack of choice of doctor and medical institution, and the decrease in trust and respect for medical personnel. The length of medical worker's residence in the rural areas can become a factor in the formation of the patient-centric approach in the medical service system and serve the development of health-saving practices of rural inhabitants. The Russian modern medical service system hinders the transition from disease-centric to patient-centric orientation, despite the proclamation of new standards and norms for healthy lifestyle. The medical worker remains a mentor for the patient and practically does not participate in the development of culture of the health of rural inhabitant.
